That’s why our ACL rehab process is built around a few non-negotiable priorities that set the foundation for everything that follows.
Control the Environment: Pain & Swelling
Swelling acts like a neurological shutdown signal to your brain, telling your quadriceps to stop working — a phenomenon known as arthrogenic muscle inhibition.
While some swelling is expected after surgery, excessive or prolonged swelling slows progress and limits strength recovery. At Echo, we track swelling at every visit and dose activity carefully so movement helps recovery rather than sabotaging it.

Battle for Extension (Straightening)
If you take away only one goal in the early post-op phase, let it be this: get your knee straight.
Restoring full knee extension early is one of the most critical factors in preventing long-term issues such as quad inhibition, altered gait, and early joint degeneration. A knee that stays slightly bent leads to scar tissue, a persistent limp, and a quadriceps that never fully recovers.
Without full extension, you won’t be the athlete you want to be — no matter how strong you get later.
Wake Up the Quad
ACL surgery doesn’t just weaken the quadriceps — it turns it off.
If you can’t restore quad activation early, returning to sprinting, jumping, and cutting becomes nearly impossible. That’s why early quad re-education is a top priority at Echo.
We use neuromuscular electrical stimulation (NMES), targeted quad sets, and biofeedback strategies to restore proper recruitment.
Gradual Flexion (Bending)
While extension comes first, we don’t ignore knee flexion.
Using progressive, low-stress range-of-motion strategies, we restore bending without compromising the graft. Typical early goals include reaching approximately 90 degrees by week one and full motion by weeks six to eight.
Patellar Mobility
Your kneecap needs to glide freely for your knee to move well.
Scar tissue can restrict patellar mobility, limiting range of motion and increasing discomfort. We address this immediately with manual mobilizations and teach athletes how to maintain patellar motion independently at home.
Restoring a Normal Gait
Rehab isn’t about ditching crutches as fast as possible — it’s about using them strategically.
Crutches help manage swelling, control load, and prevent compensatory movement patterns. Moving away from them too early often delays recovery rather than accelerating it.
